I confirm that, where required for competition, I am (or my child is) a current member in good standing with the NWBA and eligible to compete. I (or my child) will treat coaches, teammates, opponents, referees, volunteers, staff, spectators, and facilities with respect, will not approach the bench or officials during games (an NWBA rules violation), and understand there is zero tolerance for drugs, alcohol, or inappropriate behavior. I will follow all facility rules and staff instructions, and understand the Host may remove anyone for unsafe or disruptive conduct without refund.

I understand that participating in and attending wheelchair basketball activities involves inherent and other risks — including physical contact, collisions, falls, equipment failure, transfers, the actions of others, exposure to communicable illness (including COVID-19), and facility conditions — which can result in injury, illness, disability, property damage, or death. I voluntarily assume all such risks, both known and unknown, on behalf of myself, my child (if applicable), and any support person(s) with me.

In consideration of being permitted to participate or attend, I release, waive, and discharge the Released Parties from any and all claims, demands, causes of action, damages, costs, and expenses arising out of any injury, illness, death, or property loss to myself, my child (if applicable), or my support person(s), whether arising from the ordinary negligence of the Released Parties or otherwise, to the fullest extent permitted by law. If signing as a parent/guardian of a minor, I make this release on my own behalf and, to the fullest extent permitted by law, on behalf of the minor and the minor’s heirs, representatives, and family.

I agree to indemnify, defend, and hold harmless the Released Parties from any claims, losses, liabilities, costs, and expenses (including reasonable attorney’s fees) arising out of my, my child’s, or my support person(s)’ participation in or attendance at the tournament, or any violation of this agreement.

In the event of an accident, injury, illness, or emergency, I consent to the Released Parties securing emergency medical care, transportation, and treatment for myself or my child as deemed necessary, and I am responsible for all related costs. I authorize over-the-counter medication for my minor child as deemed appropriate by emergency or medical personnel.

I understand the Host and Released Parties are not responsible for any lost, stolen, or damaged personal property, including sport wheelchairs, equipment, and belongings.

I consent to photographs, video, audio, and likenesses of me or my child captured at the tournament being used by the Host for promotional, social media, fundraising, and team purposes, without notification or compensation, and I release all related claims.

This agreement is governed by the laws of the State of Idaho, with exclusive jurisdiction and venue in Idaho courts. If any provision is unenforceable, the rest remain in effect. I confirm I am at least 18, or that I am the parent/legal guardian of the minor named above with authority to sign on their behalf. I have read and understand this agreement and agree to it voluntarily.
